The Access Request Form is the form you are required to complete to apply for the NDIS. You may also be required to provide some supporting documents/information to apply for the NDIS.

If you currently get disability supports and the NDIS is available in your area, you will be contacted by the NDIS. If the NDIS is available in your area and you don’t get any disability supports, you can call 1800 800 110 and make an Access Request over the phone. Alternatively, you can complete the form and submit it via email.

If you are not happy with your provider, you can change. Each provider have different notice periods, so check your service agreement. Let your provider know and they will let you know what you need to do. If you have already selected a new provider, contact them and they will be able to help.
